What Living Well Does

Living Well in North Yorkshire aims to improve the health, wellbeing, and independence of adults. The service focuses on:

  • Providing free, targeted support for those who need it.
  • Promoting independence and self-help.
  • Working with individuals rather than doing everything for them.
  • Helping with self-assessments and referrals as needed.
  • Supporting existing services without duplicating them.
  • Offering practical advice and information.

Living Well Coordinators work one-on-one with individuals to:

  • Identify what is important to them and what support they have.
  • Help them make simple changes to their lifestyle and home.
  • Identify and remove barriers to wellbeing and independence.
  • Support them in maximizing their health and accessing local services.

They can also connect people with services like home adaptations, friendship clubs, and healthy living advice.

Living Well Coordinators are active in the community, providing feedback on local support quality and availability. They work with the Stronger Communities team to identify community needs and resources and build connections with other services like GPs and pharmacies.
